SHPE Membership Application Form

Information

  • Applications are due ASAP in Room 208 (Mr. Boswell).

 

  • Along with your applications, please attach a copy of your virtual counselor (graduation information) and receipts from dues and SHPE national

  • Contact us before paying!              9542536695

 

  • When asked for your school, make sure you find and choose the option: "Cypress Bay High School"

 

  • For the part when it asks for your GPA, please put your weighted and not your unweighted.

 

  • Once you reach the membership page, choose the 5$ SHPE Jr membership and then select our Cypress Bay chapter. Complete the transaction and print out the receipt

 

  • Make sure you press the little "x" on the box for the $2 tax-deductible donation, of course, if you wanted to make the donation then leave the box.
